Scheduling Your First Appointment

To schedule your new patient appointment, call us at (727)-362-6866 and our New Patient Coordinator will send you a secure link, which will allow you to easily complete your paperwork electronically – no printing required.

You will be receiving an email invitation from noreply@heno.io with a link and access code for your intake forms. It is required to completely fill out all the consent and intake forms prior to your scheduled visit. At times, this email may go to your spam folder, so please check there if you do not see the email come into your inbox.

Please have the access code available when you arrive in case we need to access in office for any missing required information not filled out in advance. Please be aware that if your registration process is not complete upon arrival, this may affect the amount of time you have available with your therapist. Therefore, if you have not completed your forms or they are incomplete, please arrive 30 minutes prior to your scheduled appointment time. Otherwise, arrive at least 5 minutes before your appointment start time.

  • Bring or wear comfortable workout clothing so you can easily move during your treatment.
  • Bring any diagnostic reports and prescription information from your physician, if available.

Will my insurance cover my physical therapy sessions?

Back 2 Normal Physical Therapy strives to provide the highest quality patient care. As health care costs continue to rise, Back 2 Normal Physical Therapy makes every effort to maintain our standards of care. In order to honor this commitment, we are an out-of-network provider.

We recommend clients contact their insurance company to find out what their out-of-network benefits include, so they know what to expect regarding reimbursement.

Here are some questions for them to ask:

  • Do I have out-of-network benefits?
  • Do my out-of-network benefits include physical therapy?
  • What is my out-of-network deductible?
  • Once my deductible is met, what percentage is reimbursed?
  • What is my out-of-pocket amount for my out-of-network benefits?
  • What is the effective date of my policy?
  • Do my claims run on a calendar year or do they run based on effective date?
  • Are there any forms I need to complete online and fax in with my invoice?
  • Clarify the mailing address that the claim needs to be sent to.
  • Any other requirements needed for reimbursement?

Clients may need to complete forms from their insurance company ( see above questions to ask the insurance company) and mail in with the above invoice and insurance cover letter.

Our clients are responsible for payment at the time of their therapeutic service. Back 2 Normal PT accepts all forms of payment. As a courtesy, Back 2 Normal Physical Therapy will provide you with an invoice for all services rendered.

We accept limited PIP claims to better service our clients’ needs when involved in an auto accident. Please contact our office to find out if you are eligible to use your auto benefits.

cancellations

Appointments may be canceled by calling (727) 362-6866 at least 24 hours in advance. As a small business operation, we are significantly affected by no shows and late cancellations. Maintaining goodwill is very important to us, and we do our best to accommodate unexpected changes in our client’s schedule. No-shows and cancellations with less than 24 hours’ notice prior to scheduled arrival make it difficult for us to reschedule those appointments. Therefore, no-shows and late cancellations will be charged the full cost of the scheduled appointment.
